Southwest Dental Plan is not insurance.  It is a dental plan in which participating dental providers have agreed to provide plan members with dental services at reduced fees.

Membership Costs

	Member only = $56 per year
	Member + 1 dependent = $86 per year

*Membership fees include a nonrefundable $10 administrative fee.

One dollar of every membership fee is donated to the Homeless Shelter for Children.
